In Network:

I strive to assist my clients with their massage insurance claims whenever possible. However, If you plan to use insurance, It's your responsibility to verify your insurance coverage, benefits, and requirements prior to your appointment. I am in network with Blue Cross Blue Shield Regence, Premera, Providence. I accept HSA, FSA and out of pocket payments.

Please be aware that you are financially responsible for 100% of the cost of services provided if your insurance company denies payment for any reason, including but not limited to:

  • Lack of insurance coverage for massage therapy
  • Failure to obtain prior authorization
  • Exhaustion of benefits
  • Non-medical necessity determination

By receiving services at WELLNESS BY MARISSA LLC, you acknowledge and agree to this policy. Here is a step by step guide to check your massage therapy benefits:

How to Check Your Insurance Benefits for Massage Therapy

Step 1: Locate Your Insurance Information

Gather the following details from your insurance card:

  • Insurance Company Name ( Regence, etc.)
  • Member ID Number
  • Group Number (if applicable)
  • Customer Service Phone Number (typically on the back of the card)

Step 2: Call the Customer Service Number

  • Dial the Customer Service or Member Services number on your insurance card.
  • When prompted, choose the option for benefits and coverage.

Step 3: Ask the Right Questions

When you speak with a representative, ask these specific questions:

  3. Do I need a referral from my primary care provider (PCP)?
  4. Is prior authorization required before receiving massage therapy?
  5. How many sessions are covered per year?
  6. What is my copay, coinsurance, or deductible for massage therapy?
  7. Are there any exclusions or limitations on my massage therapy coverage?
  8. Are there any out-of-network benefits if Wellness by Marissa is not in-network?

Step 4: Write Down Your Benefits

Take notes during the call. We recommend writing down the following:

  • Representative’s Name:
  • Date of Call:
  • Coverage Details (copays, number of visits, etc.):
  • Referral/Authorization Requirements:
