Appointments are reserved exclusively for you. Because time is set aside specifically for you, the following cancellation and no-show policy applies to all scheduled appointments. By scheduling an appointment, you acknowledge and agree to the terms set forth below:
LATE CANCELLATIONS:
Cancellations made less than 24 hours prior to the scheduled appointment time will incur the following late cancellation fees:
NO-SHOWS:
Failure to arrive for a scheduled appointment without notice will be considered a no-show, and the full cost of the scheduled service will be charged. Applicable fees will be charged to the card on file. Thank you for respecting my time and allowing me to provide focused, uninterrupted care to all clients.
INSURANCE & PAPERWORK RESPONSIBILITY (IMPORTANT)
I'm in network with Regence, Premera and Providence. If you are scheduling an appointment with the intention of using insurance, all intake paperwork and insurance information must be submitted at least 24 hours prior to your scheduled appointment time.
If paperwork is submitted within 24 hours of your appointment, and it is discovered that:

You are always welcome to keep your appointment and pay out of pocket at the time of service if your insurance is not accepted or does not apply.
